If you are a new patient to Pick PT, let us guide you through your first visit so you can be prepared when you get to our clinic. We want you to feel like you belong at Pick PT, so if you ever have any questions during your visit, don’t hesitate to ask our friendly staff.

We will help you determine when you arrive if you have any co-payment, deductible or co-insurance requirements. Please have your insurance card, claim information, and script available. We work with many private insurance carriers, auto insurance carriers, commonwealth carriers, and workers’ comp.

Pick PT wants to assure that your first appointment has no unexpected delays. Please arrive 15 minutes before your first scheduled appointment. Please fill out the online form below and download the medical intake and consent forms and fill them out. You can bring them in or e-mail them for your first appointment. This will save time when you arrive at our office.
